Agreement to Terms

By using the Personalised Support Systems website or engaging our services, you agree to these Terms and Conditions. If you do not agree, please do not use our website or services.

Our Services

Personalised Support Systems provides NDIS disability support services including:

  • Individualised (1:1) support
  • Group programs
  • Allied health services

Services are provided in accordance with your NDIS plan and service agreement. Specific terms relating to your support will be outlined in your individual service agreement.

Service Agreements

Before commencing services, participants (or their representatives) will enter into a service agreement with Personalised Support Systems. This agreement outlines:

  • The supports to be provided
  • Fees and payment terms
  • Cancellation policies
  • Rights and responsibilities of both parties

Please read your service agreement carefully before signing.

Cancellation Policy

Cancellations are subject to the NDIS Pricing Arrangements and Price Limits:

  • 7 clear days notice required for cancellations of disability support worker services
  • 2 business days notice required for allied health services
  • Short notice cancellations or no-shows may be charged at the agreed rate

Clear days means complete days excluding both the day you give notice and the day of the scheduled support.

Fees and Payment

Fees are charged in accordance with the NDIS Price Guide. Payment terms depend on your plan management type:

  • NDIA-managed: Claims submitted directly to the NDIA
  • Plan-managed: Invoices sent to your plan manager
  • Self-managed: Invoices sent directly to you for payment

Details of fees and payment terms are outlined in your service agreement.

Your Responsibilities

As a participant or representative, you agree to:

  • Provide accurate information about your support needs
  • Notify us of any changes to your NDIS plan or circumstances
  • Treat our staff with respect
  • Give appropriate notice for cancellations
  • Pay any fees owed in accordance with your service agreement
Our Responsibilities

Personalised Support Systems agrees to:

  • Deliver services as outlined in your service agreement
  • Treat you with dignity and respect
  • Protect your privacy and personal information
  • Respond to feedback and complaints promptly
  • Comply with NDIS Practice Standards and Code of Conduct

These Terms and Conditions are governed by the laws of Victoria, Australia. Any disputes will be subject to the jurisdiction of Victorian courts.
